Agree the reporting period, responsible owner and intended decisions before building one large form for every situation.
Keep questions relevant to the respondent and distinguish required answers from optional comments. Rating scales need a shared meaning, while open responses need an appropriate review process. Clear instructions and mobile-friendly forms help participants understand what information is being collected.
Anonymous, confidential and named feedback are different arrangements. The institution should choose which applies and explain it clearly. Reporting and access must follow that choice; a system should not promise anonymity while quietly exposing identities through exports or small-group breakdowns.
Response counts, response rates and time periods give scores context. The reporting scope should identify useful comparisons and avoid treating a small sample as a complete picture. Agree who can view individual comments and who receives only a summary of the findings.
